Print out this page and the next 2 pages and follow these 5 easy steps to registration 1.Take pictures - If the donkey is over 6 months of age the registry needs a photo of the left side and the right side. The photo can be regular prints, Polaroid or digital. These photos are placed on the back of the registration for identification purposes. These do not have to be the best of pictures, but it is important that they show a full view of each side. 2. Complete and sign the self-inspection form and determine which category you wish to register your miniature donkey. 3. Fill out this application form and all of the pedigree information available, or attach copies of sire & dams papers. Pedigrees are not required for registration, but if they are known they should be indicated. When unknown put untraced. 4.Complete the owner and breeder information on this form exactly as you want it to show on all registration certificates. 5. Enclose all of the above along with the appropriate fees, or credit card info and send it to the address listed on the home page. Owner/Applicant: Membership number (If you are a current member) ________________________ Please complete ownership information exactly as it is to be shown on registration papers.
